Maximise Clinical Impact

Our mission is to ease the pain of a chronic diagnosis so you can spend those important moments with family

Our mission is to improve patient care and increase awareness of medical conditions by facilitating access to relevant medical information and establishing a community hub. This hub will enable patients to share their experiences and connect with others facing similar challenges, thereby addressing the fragmentation within the healthcare system. This strategy aims to empower and educate patients, simplifying their navigation of healthcare resources, particularly for those residing in remote rural communities. Additionally, it seeks to expand access to and understanding of both conventional and complementary treatment options.

For example, Robert was recently diagnosed with dementia and was eager to know more about this illness and possible treatments. By using chronicaidcare, Robert was able to access recent articles on dementia. He was also able to connect with other patients who have a similar diagnosis.

Robert read several articles on the impact of dementia and possible treatment options, which helped him develop a set of questions to ask his doctor during his second consultation. By doing this, he was able to assure himself that the treatment path suggested by his doctor was the most appropriate way forward. Through the Community Hub, he made contact with several people in similar situations. They were able to reassure him that he would still have a good quality of life and also provided him with the names of several health professionals, such as physiotherapists and dieticians who are experienced in the rehabilitation process for his condition.
